The NB 15 on-ramp and GHP, sediment build up was observed at the upgradient side of the DI at the sandbags and was overtopping the sandbags. A small check dam, placed approximately 15-20 feet upgradient of the DI, was composed of straw wattles and sandbags to control the amount of sediment flowing toward the DI. Sediment build up was also observed at the straw wattle check dam. These BMP¿s did not appear to be properly maintained to limit or reduce sediment flowing onto the DI.

The NB 15 at the GHP UC, BMP¿s did not appear to be adequately maintained. Erosion had occurred on the east side of the NB 15 at the GHP UC, allowing sediment to flow under straw wattles and outfall into a nearby drainage swale.

The south embankment of the GHP UC, straw wattles and sandbags were not maintained. Sandbags (or gravelbags) were torn or deteriorated. Straw wattles were displaced and others had lost their form (flattened) to be effective as a BMP. Soil erosion on this UC embankment had accumulated at the toe of the slope and had overtopped the wattles and sand/gravel bags.

Complaint inspection: The tracking/discharge of dirt/sediment is coming from construction grading at the athletic field adjacent to the completed Academy (school) Buildings. There were grading machines working the site. I did not speak to anyone, I just took photos from the gate/perimeter. The street entrance to the active construction site is on Community Park Pointe off of Village Center Loop Road. Construction was originally scheduled for completion 12/31/13, but a COI was submitted changing completion to 03/01/2014 with revised total disturbed from 12.2 acres to 25.6 acres, with R value revision from 10.77 to 20.1. This is probably for the athletic field.Will call/email Mr. Thornton and advise him of tracking violations.

Inspection to hand deliver Notice of Violation that was returned mail-undeliverable. Fred Pedroza signed for the receipt of NOV. Charlie Horn-landlord was also onsite. The only corrective action that has been completed since the last inspection in Sept 2013 is removal of the two 55 gal oily drums. Process materials still running off site and onto Miller's Towing to the west. Material still discharging into creek bed. Inspector requested that the material be manually removed from the creek bed and that Pedroza implement structural BMPs to prevent runoff of process materials from site. SWPP needs to be fully developed and implemented for current and planned site changes/improvements. Left copies of erosion/sediment control bmps maual with Mr. Pedroza & Mr. Horn. Requested immediate correction of persistent violations or referral to COG for enforcement since two months has passed without corrective actions.

Also requested that Pedroza implement structural BMPs to prevent runoff from site while not increasing velocity during heavy rainfall. NOV should be issued for the discharges off site and into the creek. SWPPP and map onsite. SWPP needs to be fully developed for current and planned site changes/improvements. Left a copy of the IGP with Mr. Pedroza. Inspectors were told that the owners of 15275 Olde Hwy 80 are Dan Miller and Charlie Horn (owner Horn Auto & RV).

Process sand, gravel and concrete dust, as well as torn bags of Portland cement are exposed with potential of running off of site during storm events along fence where there are gaps in the block & mortar curb installed. Block & mortar curb installed along perimeter of site is inadequate (has gaps), and tarps installed as overhead/roof cover for outdoor processes and process materials are inadequate. Inspector advised Sally Smithwick that the tarps and perimeter block & mortar curb are inadequate and requested corrective action.

I provided Kelli Groeger of admin staff with a copy of the recertification guidance sheet, WDID number and Secret Code Number during the inspection. Also advised of the requirement to upload an updated SWPPP (Storm Water Pollution Prevention Plan) and site map to SMARTS no later than August 14, 2015. Noted that the facility has deficient BMPs: lack storm drain inlet protection and sediment & debris around storm drain inlets. Sent SEL BMP corrective actions email on 07/03/2015..

First site inspection. Construction approx 10% along. 3 phases of construction: Admin Bldg Site (underway), Maint Bldg Site (underway), and Bldg at Fuel Station (not started). At Admin Bldg site storm drain inlet has filter fabric but needs perimeter control- ie: gravel bags around inlet, and perimter controls needed at gate/fence line on Main St side. Storm Drain inlet on Main Street needs re-install of inlet protection. At Maint Bldg site, secondary containment and cover is needed for 55 gallon drums, pallets of chemicals-concrete cure, and bags of Surflex. Also at Maint site, perimeter control wattles need to be anchored per the SWPPP specs, and concrete washout BMPs implemented daily. No discharges were noted. The SWPPP, Maps and Visual Inspection logs are all onsite and complete. Alegre-environmental is the consultant for the project.Whiting-Turner Contracting is the general contractor.

Also, requested that they properly dispose of hazardous waste containers, and do not place these containers in general trash dumpsters I reviewed the facility SWPPP (Storm Water Pollution Prevention Plan) and found that employee training records were missing. Requested that discharger train all employees on the SWPPP BMPs and record the training in a written log including employee names, employee signatures, dates, and description of material covered. Provide a copy of the employee training log via email by June 30, 2015. Maintain a copy of all employee training logs in the SWPPP binder.

Broken gravel bags in gutter/street on southern side of site. Reviewed SWPPP and QSP site inspection logs - ok. Job foreman (Martin) walked the site with me. Project Mgr- Donna Manning arrived at end of inspection. I verbally requested that all of the aforementioned deficiencies/violations be corrected immediately. Ms. Manning agreed and said that she would send photos of corrections. Send SEL Email.
