While conducting inspections of nearby facilities on 11/20/2014, I observed (from my vehicle on the street) tracking of materials (concrete fines) onto Industry Road (parallel to Hwy 67) from the Diamond Concrete facility located at 10124 Channel Road, Lakeside. I took photos of tracked material from the Diamond Concrete site. I emailed the photos to Jaimy Dupleich, advised her that tracking of material offsite is a violation of Diamond Concrete¿s enrollment under the Industrial Storm Water permit and the terms and conditions of the permit. I requested that Diamond Concrete take corrective actions to clean up the material that has been tracked offsite, as well as implement BMPs to prevent tracking and/or runoff of process materials from the site.

Drive by on way to another site I observed heavy tracking of concrete fines and slurry offsite and onto the street. Went to office and spoke to Jaimy Dupleich. She stated that the slurry reclaimer machine was malfunctioning and that last week the truck that hauls slurry broke down and that is why there is tracking. I requested that the tracked material be cleaned up immediately.

SDWB inspector site visit on August 23, 2012 to drop off the 2011-2012 Annual Industrial Storm Water Report forms for Mr. Lenzini to complete and submit by August 31, 2012. The inspector took 8 photos during inspection.The photos show areas of concern, where Housekeeping and Best Management Practices (BMPs) need to be implemented. The areas of concern are: 1- Metal fines/shards on the ground at various locations around your facility. 2- 55 gallon drums of thinner, etc that are sitting outside without cover and without secondary containment. 3- Storm drain inlets do not have adequate structural BMPs in place and are surrounded by debris including metal shards at some drains. Inspector requested via email on August 24, 2012 with photos attached that Mr. Lenzini take corrective action on the areas of concern by August 31, 2012 and submit photos via email showing corrections as follows: 1- Sweep up/ vacuum up all metal fines/shards from the facility grounds including at the dumpster, in concrete cracks, drainage areas and around drain inlets. 2- Move the 55 gallon drums to an indoor storage location and provide secondary containment for the drums. 3- Install structural BMPs at storm drain inlets (filters, berms, gravel bags) as necessary to prevent process wastes and debris from entering the storm drains. 4- Train employees on Housekeeping and BMPs.

Storm event during inspection. Discharges of sediment to MS4 observed. Requested Mr Jones to take corrective action immediately on the structural BMPs that failed (resulting in sediment discharges to the storm drain) during the storm event that began this morning. Photos taken and emailed to Mr. Jones. The storm drain inlets that were receiving sediment laden storm water are the inlet at Cobble Creek and Bacara (receiving sediment laden water from lot 513 and an occupied lot across the street), and, the storm drain inlet on Sycamore Trail downstream of the corner where lot 292 is located. Sediment was being transported via a curb core at lot 292, as well as sediment being carried by storm water from multiple failed BMP areas upstream (up the street from lot 292). Requested that Mr Jones email photos of all areas once cleanup, and corrective and enhanced structural BMPs are in place. Advised Mr Jones that discharges of sediment are violations of the terms and conditions of the construction storm water permit (Order No. R9-2009-0009-DWQ and subject to enforcement action.
