2013-03-13	Jonas Salk Elementary 7825 Flanders Drive, San Diego California 92126	1. The project was identified as Risk Level 1. The site discharges to Carroll Canyon Creek, which is tributary to Los Penasquitos Lagoon. Los Penasquitos Lagoon is listed on the 303(d) List as impaired for sedimentation. The project indirectly discharges to a water body that is listed on the 303(d) list as impaired by sediment. Carroll Canyon Creek has also been designated with the COLD beneficial use in the Basin Plan. The Risk Level determination is not accurate. 2. The SWPPP did not include the LRP¿s signature or certification of the SWPPP. The SWPPP also did not have a reference to the training logs/records, whichare kept in a separate location. The SWPPP that is on SMARTS does not have the current QSD¿s information. 3. The SWPPP indicated that several types of BMPs were to be implemented for erosion and sediment control that were not implemented (i.e. silt fence along entire project perimeter, stabilization of recently graded areas with straw mulch). 4. Several areas require additional erosion and/or sediment control BMPs, especially along the perimeter of the site, and several slopes along the northern and northeastern parts of the site.
2013-03-13	Jonas Salk Elementary 7825 Flanders Drive, San Diego California 92126	1. Inadequate implementation of Risk Level 2 erosion controls. Inadequate implementation of erosion controls for active and inactive areas. Observed partial implementation of erosion controls (i.e. bonded fiber matrix) on "joint use field" which was identified as an inactive area requiring erosion control during the October 7, 2013 inspection. Observed several areas that were inactive or should have been scheduled to be inactive without adequate erosion controls. 2. Inadequate implementation of Risk Level 2 sediment controls. Lack of linear sediment controls along toes of several slopes. 3. Inadequate implementation of housekeeping BMPs. Observed lack of covers for several trash receptacles, lack of preventative measures for concrete waste handling, lack of cover and containment for waste and soil stockpiles.

Observed water discharging from the site transporting sediment to a the municipal storm sewer system. Observed inadequate implementation of erosion control BMPs on inactive areas and slopes on the site draining toward Flanders as evidenced by sediment buildup and behind perimeter sediment control BMPs. Perimeter sediment control BMPs were ineffective in preventing storm water and pollutants in storm water from discharging from the site, as evidenced by sediment and sediment laden water observed on sidewalk and gutter leading to storm drain inlet on Flanders. See photos attached to Inspection ID 2021545.

2013-03-05	91 HOV Gap Closure State Route 91, Riverside California 92522	¿ Storm water runoff from SR-91 ponded on the top of the slope along the EB SR-91, and the University Street off-ramp (near the intersection of Mulburry Street and 9th Street), causing the slope to fail, and sediment discharged into a storm drain inlet associated with Drainage System No. 123. The contaminated storm water runoff passed through the drainage system, under SR-91, and discharged out to 10th Street, on the north side of the WB 91. From 10th Street, the contaminated storm water runoff drained down the street and ultimately into a City of Riverside storm drain inlet near the intersection of 11th Street and Lime Street

Silt, sediment, and mulch contaminated storm water runoff discharged into a storm drain inlet on NB Madison Street
