Recycled water trench has little freeboard and is exposed to storm water. Recycled water trench needs to be cleaned out.

Process sand, gravel and concrete dust, as well as torn bags of Portland cement are exposed with potential of running off of site during storm events along fence where there are gaps in the block & mortar curb installed. Block & mortar curb installed along perimeter of site is inadequate (has gaps), and tarps installed as overhead/roof cover for outdoor processes and process materials are inadequate. Inspector advised Sally Smithwick that the tarps and perimeter block & mortar curb are inadequate and requested corrective action.

During my inspection I determined that Fibre Resources, sorts, bails- mechanically and, ships/sells cardboard & paper (newspapers/shredded documents)I observed two storm drain inlets onsite that were not adequately protected (lacking structural BMPs). I will address that issue with them in an SEL e-mail with photos.

Visited facility on July 2, 2015, to advise discharge that they are required to recertify in SMARTS for enrollment under the new Industrial Storm Water Permit (Order No. 2014-0057-DWQ) no later than August 14, 2015. I provided Kelli Groeger of admin staff with a copy of the recertification guidance sheet, WDID number and Secret Code Number during the inspection. Also advised of the requirement to upload an updated SWPPP (Storm Water Pollution Prevention Plan) and site map to SMARTS no later than August 14, 2015. Noted that the facility has deficient BMPs: lack storm drain inlet protection and sediment & debris around storm drain inlets. Sent SEL BMP corrective actions email on 07/03/2015..

Straw wattles on slopes along Airway Road worn down, rolling down hill into storm channel and need replacement. There is no Storm Drain Inlet protection at 2 storm drains on Airway Lane. Photos taken. Need more structural BMPs for perimeter controls, erosion & sedimentation controls, and need to install storm drain Inlet protection. Silt fence is properly installed along sidewalk perimeter on Airway Lane, however, there is loose soil/sediment on the sidewalk side of the silt fence and requires additional perimeter controls to prevent transport of soil\/sediment during rain events. Will send inspection report via email to Discharger requesting corrective actions.

First site inspection. Construction approx 10% along. 3 phases of construction: Admin Bldg Site (underway), Maint Bldg Site (underway), and Bldg at Fuel Station (not started). At Admin Bldg site storm drain inlet has filter fabric but needs perimeter control- ie: gravel bags around inlet, and perimter controls needed at gate/fence line on Main St side. Storm Drain inlet on Main Street needs re-install of inlet protection. At Maint Bldg site, secondary containment and cover is needed for 55 gallon drums, pallets of chemicals-concrete cure, and bags of Surflex. Also at Maint site, perimeter control wattles need to be anchored per the SWPPP specs, and concrete washout BMPs implemented daily. No discharges were noted. The SWPPP, Maps and Visual Inspection logs are all onsite and complete. Alegre-environmental is the consultant for the project.Whiting-Turner Contracting is the general contractor.

The BMP violations include improperly stored and exposed engines and transmission cores, and improperly disposed hazardous waste containers in a trash dumpster. Requested discharger to correct these violations and implement housekeeping and structural BMPs to provide secondary containment and eliminate exposure of engines and transmission cores to storm water no later than June 15, 2015. Also, requested that they properly dispose of hazardous waste containers, and do not place these containers in general trash dumpsters I reviewed the facility SWPPP (Storm Water Pollution Prevention Plan) and found that employee training records were missing. Requested that discharger train all employees on the SWPPP BMPs and record the training in a written log including employee names, employee signatures, dates, and description of material covered. Provide a copy of the employee training log via email by June 30, 2015. Maintain a copy of all employee training logs in the SWPPP binder.

Broken gravel bags in gutter/street on southern side of site. Reviewed SWPPP and QSP site inspection logs - ok. Job foreman (Martin) walked the site with me. Project Mgr- Donna Manning arrived at end of inspection. I verbally requested that all of the aforementioned deficiencies/violations be corrected immediately. Ms. Manning agreed and said that she would send photos of corrections. Send SEL Email.

Site lacks adequate housekeeping BMPs, sediment and perimeter control BMPS are deficient. Erosion control BMPs deficient on slope at south east corner of the site. Storm Drain inlet on Palm Ave at corner of Beyer requires inlet protection. The inspector verbally requested that Kevin Steiner, Job Supt and Mike Hagler, QSP to correct all BMP deficiencies. Will send an email as SEL to correct all BMP deficiencies. There is a sump inlet on the southeast section of the site adjacent to the bldg construction that requires replacement of the filter fabric and cleanout. It was clogged with sediment. The sump inlet drains to a 3 stage clarifier which drains to an infiltration pit. I requested plans/spec sheet on the clarifier system. Also requested proper labeling and or disposal of black 55 gal drums on the northern side of the site where equipment staging is located.

Site soils were not stabilized, soil exhibited signs of recent grading activity, and there were no BMPs at the site. According to the construction site plan contained in the SWPPP, page 113, the area observed was not intended to be graded.
