¿ Storm water runoff from SR-91 ponded on the top of the slope along the EB SR-91, and the University Street off-ramp (near the intersection of Mulburry Street and 9th Street), causing the slope to fail, and sediment discharged into a storm drain inlet associated with Drainage System No. 123. The contaminated storm water runoff passed through the drainage system, under SR-91, and discharged out to 10th Street, on the north side of the WB 91. There does not appear to be a separate WDID Number for the borrow site. The site is vegetated with weeds and appears to be stable. There is water accumulation in two ponds at the site. This may need to be pumped to comply with vector control requirements.

During the inspection we noted several areas where the requirements of the CGP are not being adequately implemented: 1. The perimeter control BMPs were not adequately implemented around the project site. No perimeter control BMPs were present on the north side adjacent to the shopping center, or on the south side adjacent to the vacant lot. This is a violation of Section E.1 in Attachment D to the CGP. This deficiency was also noted in the October 24, 2013 inspection report, which means over four weeks have passed without correction of this deficiency. 2. There were inadequate erosion and sediment control BMPs implemented for the areas under active construction, which are required for Risk Level 2 construction sites under Section E.3 in Attachment D to the CGP. This deficiency was also noted in the October 24, 2013 inspection report, which means over four weeks have passed without correction of this deficiency. 3. There were no linear sediment controls observed along the toes or grade breaks of exposed slopes, which are required for Risk Level 2 construction sites under Section E.3 in Attachment D to the CGP. 4. Loose stockpiled construction materials that were not actively being used during the inspection were observed without cover or berm. This is a violation of the construction materials management requirements of Section B.1.b in Attachment D to the CGP. 5. There was trash observed on the ground along the east side of the site. There were no trash receptacles observed. This is a violation of the waste management and non-storm water management requirements of Sections B.2.a and C.1 in Attachment D to the CGP. 6. The SWPPP is inadequate and appears to be based on a Caltrans SWPPP template developed for the previous CGP (Order No. 99-08-DWQ). The SWPPP does not include the BMPs that are required for a Risk Level 2 site, which is a violation of Section XIV.B in the CGP.

Per report by City of Stanton, K&G cleaned a spill of wet concrete using water which was discharged to the storm drain. An interview with Kobe Pham confirmed the event. Mr. Pham stated that most of the material was recovered but the residual was washed off the street. City photos show significant cementitious powder in the gutter and residue in the storm drain.

Fiber rolls acting as a perimeter control on the Site's northern property line were implemented exclusively by gravel bag weighting. As per the CASQA Construction BMP Handbook, Fiber Rolls (SE-5) are "not effective unless trenched," and are to be installed by staking and are to not be abutted when multiple rolls are placed in a roll. The Site SWPPP references CASQA guidelines for SE-5 installation. Therefore, the site failed to effectively implement perimeter controls.
